Enrollment Forms, Immunization, and Health Requirements:
To comply with state standards, all enrollment papers and health forms must be completed before attendance. All preschool children must have current immunization records on file at the center.
Parents need to inform the center of any changes in address, phone number, employment, emergency contact information, or changes in family situations.
Teachers are not required to have shot records on file, and are not required to have any specific immunizations in order to attain and maintain employment.
Please be sure to complete all authorized person information, including phone numbers. For emergency contact information, please provide name, address, and phone numbers for each person.
